What's inside CeZo Hospice Quality

One upload feeds the whole quality loop — intake, audit, readiness, and submission pre-flight. Every compliance answer is rules-based, with the evidence to back it.

Chart intake

AutoChart™ — upload the chart, review instead of retype

AutoChart™ is the chart-reading feature inside CeZo Hospice Quality. Upload one chart PDF — even a scanned one; the OCR runs locally — and it auto-populates the patient tables, KPIs, the Survey Ready dashboard, the 107-item CoP audit with evidence quotes, and the HOPE pre-flight. Big-batch uploads up to 2 GB and combine-to-one-PDF are built in.

  • One upload populates patient tables, KPIs, survey readiness, and HOPE pre-flight
  • Local OCR for scanned charts — processing stays on your system
  • Your quality nurse reviews the findings instead of retyping the chart

Compliance audit

107-item CoP audit with evidence quotes

Built on 42 CFR 418 — the actual hospice Conditions of Participation, across 21 domains. Every item the app marks “Met” carries the exact sentence from your chart that proves it: the same sentence you'd show a surveyor. When the app isn't certain, it says needs review instead of guessing either direction — never a false fail, never a hollow pass.

  • Evidence quote on every auto-“Met”
  • Per-patient audit ledger with gap filters
  • Agency Profile: agency-level CoP items answered once, applied everywhere

Submission readiness

HOPE iQIES pre-flight

HOPE replaced HIS on October 1, 2025. Every patient assessment now runs on a 30-day submission clock, and agencies must keep submissions at or above the 90% threshold to protect their annual Medicare payment update. The HOPE pre-flight checks your chart against those requirements before anything goes to iQIES — so problems surface while there's still time on the clock.

  • 30-day submission clock tracking per assessment
  • ≥90% APU threshold monitoring
  • Pre-submission checks, so surprises happen in-house — not at CMS

Readiness at a glance

Survey Ready dashboard

One screen that answers the 3 a.m. question: where do we stand? Readiness gauges for CMS, ACHC, and CHAP sit on top; the detail underneath is fully transparent — you can see exactly how every score is built.

  • CMS / ACHC / CHAP readiness gauges
  • Transparent SFP scoring — see exactly how the Special Focus Program score is assembled
  • Per-patient audit ledger, drill-down from the gauge to the chart line
  • CAHPS tracking

The front door

Referrals & Intake with LCD eligibility pre-screen

Catch eligibility questions at the referral stage, not at billing. Drag a referral packet in and CeZo Hospice Quality runs an LCD eligibility pre-screen against NGS J6/JK criteria — auto-checked criteria with a readiness gauge, saved to the referral record.

  • Drag-and-drop referral intake
  • LCD eligibility pre-screen (NGS J6/JK parity)
  • Eligibility result saved with the referral, ready for the admission decision

Security

Your patient data stays yours

The security stack ships in every tier — it is not an upsell.

  • AES-256 encryption at rest — records are stored scrambled; a stolen disk is gibberish
  • MFA on every account — a password alone is never enough
  • Configurable auto-lock (default 5 min) — a workstation left open locks itself; set the exact timeout to fit your facility
  • View audit trail — every look at a patient record is logged
  • Encrypted off-site backup — protected even in a disaster
  • Local-first AI — charts are processed on your system and never sent to a cloud AI; the app works fully with AI switched off
